  • Rent receipt or lease agreement from the last 30 days if renting.

  • Valid United States photo ID

  • Social Security Number

  • Valid phone number

  • Mailing address

To turn OFF your service, we will need the following items:

  • Valid picture ID for account holder

  • Valid phone number

  • Mailing address
